3. Madhyamik Admit Card/Birth Certificate

If there is any changes in admit which is not similar to aadhar in name/father’s name/address, affidavit from first class magistrate is required.

4. Marksheet of HS

5. Pass Certificate of HS

6. Caste certificate if applicable

7. Physically Challenged Certificate if applicable

8. Anti-ragging Affidavit (10/- rupees stamp paper + Online)

10. If there are any changes in name/father’s name/address, affidavit from first class magistrate is required.
